FinalForms - Parents/Guardians of MS/HS Student Athletes (7th-12th Grade)
Our district has partnered with FinalForms, an online forms and data management system for athletic and activity participation. FinalForms is an easy-to-use platform that enables parents and guardians to complete, upload, and update required athletic forms online from the convenience of your phone, tablet, or computer. FinalForms is a proud partner of the Missouri Interscholastic Athletic Administrators Association (MIAAA) and the National Federation of High Schools (NFHS).
Information carries over season-to-season and year-to-year, meaning you will never need to enter the same information twice. Whenever possible, FinalForms pre-populates data for each student—saving you time and simplifying the process.
What do we need you to do now?
At this time, if any 7th-12th grade student is planning on playing a MS/HS Winter or Spring Sport during the rest of the 2026-2027 school year, we ask that you take time now to get your parent and student accounts set up and your student-athlete's information into FinalForms in advance of the start of the upcoming season(s) and parent meeting(s).
Important Note: If the Activities Director already has your student's required MSHSAA physical form and information on file, then you will not need to upload this document or fill in this information at this time. Our Activities Director will do this for you. However, there will be some basic information that you will need to complete and you will need to designate which sports your student-athlete will be playing during any upcoming athletic seasons for the 2026-2027 school year.
This will help ensure your student is ready and able to play.
Future School Years
Going forward, at the beginning of each school year you will be required to verify that the information in FinalForms is current and electronically sign all forms. You will also select each sport your student plans to participate in during the school year. If you need to update your student-athlete's information, you are able to do so and sign the forms as needed. And, you may review your data at any time to verify it is current.
Account Setup & Signatures
This system requires both a parent/guardian and the student to create accounts. Both must log in and sign all required forms for the student to be fully cleared.
Once the parent/guardian is registered, they can:
● Create accounts for their student(s)
● Complete and sign all required forms
● Review and update information at any time
